Our services can start from just a minimal number of hours to 24x7 services and can be decreased or increased as needs arise. Ho’okele’s team includes professional clinical staff and skilled, trained, licensed and bonded personal care assistants to help you to remain independent as long as possible. Service levels Ho’okele has four in home levels of services that can meet a seniors’ needs at different times of their lives and on their terms. Concierge service level A key component of aging well is to remain socially active and involved with friends, family and other seniors. Let’s us take care of the planning, transportation and multiple details to arrange outings with friends, accompaniment on trips or cruises, attending groups meetings and activities, or a arranging a special lunch or dinner with friends. Sit back and enjoy special outings, or just someone to go to the park or shopping with you. Companion level When the housekeeping and cooking becomes a chore and it’s just too much to think about. We can customize a care coordination package to meet your needs.

ferent times of their lives and on their terms.
- Assessment and initial planning with a service plan
- Initial care coordination defined by the service plan
- Access to RN Navigator services - up to 4 hours of service over a 30-day period
• Standard Package
- o Basic Plan with access to RN Navigator services - up to 4-hours per year
- o Telephone consultation as needed
• Premium Plan
- Basic plan with access to RN Navigator services - up to 20-hours of services per year
- Telephone consultation as needed
Specialized Plans
There may be a time that alternative senior solutions need to be implemented.
Long Term Care Placement
• 8-hours of service
• If additional hours are required, the hourly navigation rate will apply
